first ran MacOs servers in-house
then [WinNT] servers in-house - in closed off room, with floor-standing [Air Conditioning] unit
big problems maintaining consistent temperature (and computers hate changing temperature even if its not to extreme levels)
Broad Band management was a pain in the butt (though more people know how to handle BGP these days)
then CoLo in Exodus (in [Jersey City]) which was a pain if you had to deal with OS failures (thanks [WinNT]) or hardware failures (e.g. Load Balancing box)
we ran our own servers in-house (Win2k) - low load, mostly intranet, so not as bad to deal with
